Miles & St. Andrews Home Health & Hospice has been named one of the nation’s top home health agencies by HomeCare Elite.
The recognition identifies the top 25 percent of Medicare-certified agencies in the United States based on publicly available performance measures in quality outcomes, best practices, patient experience and quality improvement. The award is sponsored by OCS HomeCare, National Research Corporation, the leading provider of home health metrics and analytics and by DecisionHealth, publisher of Home Health Line, an independent newsletter covering home health.
Kathy Bean, RN, Executive Director of Miles & St. Andrews Home Health & Hospice said the award is important for what it says about the quality of care that Miles & St. Andrews providers offer and the commitment of caregivers to their patients. Recent patient satisfaction surveys give the agency high marks.
As a home health provider in one of the oldest counties in the oldest state in the country, Miles & St. Andrews Home Health & Hospice has many patients with at least one, and often several chronic diseases, such as heart disease and diabetes, or chronic obstructive pulmonary disease and high blood pressure.
Over the past several years, thanks largely to the generosity of the St. Andrews Auxiliary, almost all clinical staff members have become certified in chronic disease management, said Bean. This allows for comprehensive assessments and it allows the clinicians and home care patients to set goals together.
“This gives us the ability to look at not just the reason the patient was referred but also all the other challenges they face,” Bean said.
Understanding how all of a patient’s medical challenges affect them is vital to offering the high level of care that people need to continue to stay at home. It is also helps patients make a smooth transition from the hospital to home or from a rehabilitation facility to home.
